Home Health Criteria for Admission
- Physician Orders – The physician prescribes specific orders for the home health plan of care of each individual patient.
- Homebound – An individual is unable to leave their home unassisted, and/or leaving takes considerable and taxing effort.
- Intermittent Skilled Need – An individual requires short-term, intermittent care that is provided by at least one of the following: a licensed nurse, physical therapist, speech therapist, or has a continuing need for occupational therapy.
