Home Health can be the preferred treatment for illness or injury because professional health care is delivered to the patient in the comfort of their own home.  The familiar atmosphere with family, pets and personal surroundings can be a beneficial way to restore or maintain health.

Continua Home Health services are available 24-hours a day, 7 days a week, 365-days a year.  The criteria for Home Health Admission are:

Physician’s Orders-The physician prescribes specific orders for the Home Health plan of care for each individual patient.  The plan of care includes the kind of services needed and specifies the professional designated to provide these services.

Homebound-An individual is unable to leave their home unassisted or leaving becomes a taxing effort.  An individual may leave home for medical treatment or short infrequent absences for non-medical reasons.

Intermittent Skilled Need-An individual requires short-term, intermittent care that is provided by at least one of the following; a licensed nurse, physical therapist, speech therapist or has continuing need for occupational therapy.
